I, hereby consent to receive The Shift Device™.  I understand that The Shift Device™ is a device that produces a low-frequency, high-voltage electromagnetic field, and it is based on the principles developed by Nikola Tesla.  I understand The Shift™ device is not a medical device.

I have been informed of the following:

Purpose:  The Shift Device™ is used to raise the electromagnetic field of the body for purposes of enhancing energy and promoting overall wellness.

Procedure:  The Shift Device™ involves holding the device three inches on the outside of the body, scanning the body front and back, emitting a gentle electromagnetic field.

Benefits:  The Shift Device™ may enhance energy, increase circulation, and may provide a sense of joy. 

Risks:  While using The Shift Device™ is considered safe, avoid using if you have a Pacemaker, Insulin Pump, Pain Pump, Rectal Implant, Defibrillator - ICD Device, and or anything battery operated.  Remove hearing aids when using.

The Shift Device™ device in question is not intended for diagnostic, treatment, or curative purposes without explicit consent or supervision from a qualified healthcare professional. It underscores the importance of seeking proper medical advice and not relying solely on the device for healthcare decisions.
